Dog surrender form

Thank you for filling out the form. Here is some additional information about surrendering your senior dog to Forever Loved Pet Sanctuary.

Do we require a surrender fee?

No, but while we do not require a surrender fee, we would appreciate any donation you could make towards the future care of your senior dog. As of the end of 2024, we spent on average $1,600+ per senior dog to provide medical care and everything listed below. All donations are greatly appreciated no matter how small.

What does “surrendering” mean?

When you surrender your pet to Forever Loved Pet Sanctuary, you are relinquishing ownership and responsibility of that animal to our organization. The owner surrender form you filled out allows us to complete necessary medical care and prepare the dog for adoption.

What happens after you surrender your senior dog?

We will provide the senior dog with all necessary medical examinations, vaccinations, behavior evaluations, and also provide the required shelter, food, and social interaction needed to either rehabilitate or otherwise prepare the pet for adoption. Our team includes many trusted foster parents, experienced volunteers, and partnered vets all committed to the well-being and long-term care for the senior dogs that pass through our sanctuary.

Can we provide you updates about your surrendered senior dog?

Please note that we are a largely volunteer-run organization and do not have the ability to respond to each request on a regular basis but will try our best to update you upon request. We will not disclose any contact information for your surrendered dog’s adopter or foster.
