Liability Waiver & Acknowledgment

I understand that my participation with Forever Loved Pet Sanctuary (FLPS) is on a volunteer basis. I
agree to follow the rules and guidelines of the organization.

Although FLPS maintains a safe environment for volunteers, the undersigned acknowledges that
there are risks associated with working with animals, including, but not limited to, bites, scratches,
and allergic reactions. In addition, the undersigned expressly acknowledges such risks and further
agrees to hold FLPS harmless and release FLPS from any and all claims for potential injury and/or
liability that might arise out of being exposed to such risks.

I agree to follow the supervision of all persons involved in volunteer management.

Photo & Media Release

I understand that FLPS may use my photo(s), name, and likeness on the FLPS website, FLPS
campaigns or advertisements, and any other media formats and channels utilized by FLPS. I agree to
allow FLPS to use my photo, name, and likeness as described.
