I agree that my services as a Foster Care Provider are provided strictly on a volunteer basis. I shall receive no pay, benefits, or compensation of any kinds from Forever Loved Pet Sanctuary (FLPS) for my foster care of animals. I agree to provide foster care in strict compliance with the policies and procedures of FLPS. This includes, but is not limited to the following:

Providing compassionate and loving care for whatever animal is taken into my home as long as I am able or until the animal has been successfully placed in a permanent home, whichever comes first.
Providing adequate food, water, shelter safe containment and humane treatment for the animal(s) at all times.
Providing medication and veterinary care when needed at the expense of FLPS and with their approval beforehand.
Monitoring the animal(s) and providing proper care and socialization.
Notifying FLPS within 24 hours of any major change in the health of the foster animal(s).
Immediately notifying FLPS if an animal becomes lost or stolen.
Attendance of animal(s) at adoption events.
Allow a transporter to take animal(s) to adoption events if you are unable.
Representing yourself in a professional manner.

FLPS reserves the exclusive right to determine the proper course of action to take upon notification by the foster parent of any inability to comply with this agreement.

I understand and accept that the fostered animal(s) are the exclusive property of FLPS. This agreement transfers no ownership rights. I further understand and acknowledge that I do not have any authority to keep or place the foster animal(s) in other homes or with other individuals. Moving the animal(s) or placement in permanent homes can only be done with the full knowledge and approval of the FLPS Board of Directors.

I understand that the foster animal(s) must be scheduled for needed medical treatment (altering, vaccinations, microchip, medical testing, etc.) with an approved FLPS veterinarian.

I will respect FLPS’s decision to determine if a foster parent can adopt a foster animal. I understand and accept that the procedure for adoption will be the same for the fostered animal as it would be for any other applicant. As a foster parent, I will be asked to sign the same contract and pay the same fee to FLPS that any adopter would. I further understand and acknowledge that I may not keep the animal without FLPS Board approval. The adoption fee is not final until the animal is spay/neutered, vaccinated, vet checked, all fees are paid and adoption contract is signed.

I agree to vaccinate my own animals against the following diseases before fostering: Dogs are immunized against Distemper, Parvovirus, Parainfluenza, Hepatitis (4 in 1 booster), Bordetella (kennel cough), Rabies and are free of parasites. Cats are immunized against Feline Panleukopenia, Rhinotracheitis, Calicvirus (3 in 1 booster), Feline Leukemia and are free of parasites.

If I choose not to vaccinate my animals, I understand that FLPS will not be responsible for any illness that occurs related to non-vaccinated animals.

I understand no reimbursement by FLPS will be given to me regarding my expenditure, which I incur for the care and treatment of the foster animal(s) without advanced approval.

I understand if a foster animal under my care or my own animal dies from a contagious disease, I will not be considered for fostering other animals of the same species for a specific length of time as deemed suitable by FLPS. Discussion with an approved FLPS veterinarian will determine the length of time necessary before fostering any animal again in the foster’s home

I understand that I may not rescue any animal without prior approval from FLPS.

I will ask necessary clarifying questions before taking any animal(s) into foster care. I will promptly address matters of concern to the Director of Fosters for FLPS.

I will be patient and tolerant during the transition period, which can last from a few days to a few months). I am prepared to work with the animal(s) on basic training, such as housetraining and/or litter box training. I will keep the animal(s) primarily inside the home. Dogs are not to be left outside for long periods of time and, when outside, dogs shall have shade, fresh water and food at all times.

I will make certain that the animal(s) are wearing a securely fitted collar with proper identification at all times. FLPS must be the contact name and phone number on the tag.

I will immediately notify FLPS of any problems or difficulties resulting from fostering.

I agree to keep a dog(s) leashed outside when there is not a securely fenced area. I will not tie or chain the dog(s) to anything at any time.

I will not use prong collars or employ harsh training methods with any foster animal(s) and I will never strike or choke the animal(s) in an attempt to discipline.

I will not allow unknown people/animals to approach the animal(s) without being in control.

I understand and accept that prospective adopters may wish to visit the animal(s) before adopting them. I am willing to allow people to meet the animal(s) either at the potential adopter’s home or at a mutually convenient place.

I understand and accept that FLPS makes no representations or warranties concerning the health, temperament, behavior and/or previous ownership of the animal(s).

I understand that any breach of the conditions of this foster care agreement may result in immediate termination of this agreement. In that case, FLPS shall take immediate possession of the fostered animal(s).

I agree to release, discharge, indemnify and hold harmless FLPS, including its agents and employees, for any personal injury or damages to property or pets caused by the foster animal(s).

I recognize that in handling foster animal(s) there exists a risk of injury, including physical harm caused by a foster animal. On behalf of myself, my heirs, personal representatives and executors, I release, discharge, indemnify and hold harmless FLPS, its agents, volunteers and employees from any and all claims, causes of action or demands, or any nature of cause connected with my foster care agreement.

I have received, read and understand the Foster Care Guidelines provided by FLPS.
